Have you asked him why he still does this?? What have you done to him, what has been the consquences when he does this? II only ask because he is old enough to know that he should be going in the potty.

My son was 4 years old and still doing this. I took him to a gastroentronologist who said his bowel was impacted and his colon was three times the size of a normal four year old child.
We used Benefiber (15-20 tablespoons a day between the hours of noon and 6pm) to cleanse him. After 2 months on this plan and occasionally a little bit of miralax to soften the bowels (even though they were very runny to begin with) he wasn't pooping in his pants anymore. He could feel the urge and now goes to the potty. He is regular now... by that I mean he poops at almost the same time each day because his body was trained with food and Benefiber how and when to eliminate waste - AMAZING!
What I learned from this whole experience is that my child doesn't get enough fiber in his diet which is so important. I still give him regular doses of Benefiber every day - not 15 to 20 tablespoons anymore, though.
My thought would be medical also I would check with you doctor. Another thing is to watch him. What does he do when he is doing this. Does he hide or get quite? If you notice this behavior take him to the bathroom and set him on the toilet.
Has this been an on going thing for him or did he at one time go on the potty and then reverted back to going in his pants? Definately talk to him and find out what is going on in his mind for him to do this because a change in potty habits is not only a possible medical issue sign but also a sign of distress. Whether it is physical or emotional or even that there is a bully at school bothering him.
