Both of mine I decided to do the "cold turkey" approach. I told them up front I was throwing away the pacifier, that they were "big kids" and didn't need it anymore. They cried a little the first night, but that was the end of it.
I believe cold turkey works best for the kids. It's normal for them to cry at first (even stay up the whole night during the first night screaming for the pacifier), but you have to be firm. It's also a good idea to show your child that you're already throwing away the pacifier so that he/she won't think that you're just keeping it in a drawer somewhere. It'll be hard but no permanent damage will be made
